Your child’s plan of care should be as unique as they are. For infants, toddlers, and children ages birth through five with emotional challenges, effective care shouldn’t be delayed. We understand that you want to give your child the best, but accessing services can be a challenging task for any family.

We provide your family with opportunities.

We are your guides, your allies, and your support system. We’ll help you and your family identify your strengths and your most urgent needs to develop a long-term plan to improve mental health outcomes for your child or youth. Our program is familyled and founded on unconditional care. What’s important to you is important to us.

Long-Term Care that Works

Your family’s care coordinator will use the FAST Forward planning process to get you the help you need. FAST Forward is designed to serve children with emotional and behavioral challenges whose needs are not met by traditional services. Our mission is to keep your family together and your child in their community.

We aren’t just another voice telling you what to do.

FAST Forward isn’t intended to replace existing treatments, services, or therapies. Instead, we partner with you to access the available services and supports that match your goals. Your coordinator will help you develop a plan of care and facilitate referrals to the services you need.

As part of a pilot program, Connected Families NH is able to provide FAST Forward services to children under the age of 5.
