Job Responsibilities
- Maintains a caseload of 8-10 youth and families.
- Advocates for youth and family and ensures their voices are heard.
- Ensures treatment plans are relevant and transition plans are realistic.
- Supports episodes of residential treatment and transition planning.
- Facilitates TrECC team meetings.
- Facilitates family meetings.
- Coordinates and monitors service between agencies to meet the identified individual needs.
- Works in conjunction with youth, family and existing teams and the residential treatment program by: 1) Supporting youth and families in the referral process including tracking of referrals to episodes of residential treatment. 2) Attending treatment team meetings. 3) Coordinating and making referrals to supportive community services for up to six months following transition. 4) Assisting families with applying for services or Medicaid. 5) Conducting ongoing assessments by utilizing the CANS & Preparedness Tool.
- Engages with coaching in accordance with TrECC standards
- Enters all appropriate case specific information into the data system.
- Must be independent, organized, flexible, reliable, creative, and work collaboratively as part of a team.
- Adheres to ethical, Medicaid required reporting, and confidentiality standards as required by state laws and TrECC standards
- Will work towards TrECC certification within the first two years of employment.
