Connected Families NH
Connected Families NH is a care management entity that provides support and resources for children and families experiencing mental health challenges at any stage from birth to age 21. As a care management entity, we provide a designated care coordinator who works closely with your family to create a care plan that meets your needs and goals.
The Connected Families NH process is community-based, using resources and supports within your own community. Family-driven and youth guided, we focus on staying close to your home, community, culture, family, friends – wherever you feel the most comfortable. We are your guides and your allies as you navigate mental health challenges with your child.
We serve both Northern and Southern New Hampshire, with offices in Keene and Claremont. Our services are free of charge and work with your life and schedule, making the process as stress-free as possible.
The Connected Families NH team is made up of educated, experienced, and compassionate individuals that are focused on helping you and your child reach your full potential.
