Job Description

Connected Families NH is seeking a TrECC Care Coordinator for our TrECC (Transitional Enhanced Care Coordination) program. This position serves youth aged 0-21 and their families and will involve travel around Cheshire, Sullivan and lower Grafton Counties in addition to occasional state-wide travel. Working with children, youth and families in their homes and community, the TrECC Care Coordinator will work to ensure seamless transitions for children and families leaving residential care facilities and reentering their communities. This position will also develop individualized plans and get families, community partners, and other professionals involved in a collaborative effort to empower youth to thrive during and well after their transition.

  • Minimum of Bachelor’s Degree.
  • At least two (2) years of experience, preferably in health care, social services, behavioral health, human services, or education.
  • Individuals with lived experience will be considered as well.
