Brief Enhanced Care Coordination

When a family is facing uncertainty around a child or youth’s mental health, knowing what to do next can feel overwhelming. Brief Enhanced Care Coordination (BrECC) is designed to provide short-term, focused care coordination that helps families stabilize, identify priorities, and connect with the right supports.

BrECC offers time-limited coordination during critical moments. It is designed to help families navigate systems, understand options, and prepare for what comes next. This program is not intended to replace long-term care. Instead, it bridges gaps, supports decision-making, and helps families move forward with greater support and community connections.

Determine if BrECC is the right fit

Determine if BrECC is the right fit If you have a short-term need, BrECC may be appropriate when a child or youth is experiencing increased mental health needs, and difficulty accessing timely supports. Referrals can be made by providers, systems partners, or families seeking short-term coordination.

Connect with a BrECC Coordinator

Once referred, a care coordinator reaches out to begin intake, explain the program, and outline the family's goals over the coming weeks.

Create a focused plan for support and transition

The coordinator works closely with the family to identify priorities, address urgent needs, and assist with connecting appropriate community-based mental health and family supports, adjusting the plan according to their needs.

Ongoing Check-Ins and Coordination

Throughout the BrECC process, families receive regular in person check-ins to ensure supports are working as intended and allow space to address concerns as they arise.

What a BrECC Coordinator Supports

Each family is matched with a coordinator who may:

  • Complete assessments to understand safety, support needs, and priorities
  • Identify and address gaps in community services and support needs
  • Facilitate referrals to mental health and family-based supports
  • Assist families in identifying and strengthening natural supports
  • Provide weekly check-ins to monitor progress and adjust coordination

This work is family-driven and youth-guided, with decisions shaped by what matters most to the family.

Community-Based Support

BrECC focuses on helping families stay connected within their communities. By strengthening coordination and reducing confusion across systems, the process supports families as they navigate mental health needs without adding unnecessary layers of care.

Short-Term by Design

BrECC typically lasts three to six months, depending on need. As families gain stability and clarity, the coordinator helps support a thoughtful transition to ongoing services when appropriate.
