Every child is different and requires individual care to help support them emotionally and mentally. Families who are experiencing difficulties due to mental health challenges can often struggle to find the support that meets their needs.
Connected Families NH recognizes the importance of emotional and mental health and provides family-led services and support. By bringing together a team of people to provide family-driven care coordination to support a family’s needs and the appropriate support for the family to make appropriate decisions. This team may include friends, relatives, teachers, counselors, doctors, social workers, caseworkers, and anyone else you choose to be your support system. A care coordinator will then help lead the team to support the youth in all aspects of his or her life to achieve positive emotional and behavioral health outcomes.
Here are the programs we provide to better support your family.
FAST Forward
Connected Families NH is a care coordination planning process to help you and your family identify your strengths and your most urgent needs to develop a long-term plan to improve mental health outcomes for your child and connect you with resources within your community that can help you. Our FAST Forward program is family-driven and founded on unconditional care. What’s important to you is important to us.
Your family will be set up with a Care Coordinator who will use a planning process designed to serve children and young adults with mental health challenges whose needs have not been met.
Early Childhood Wraparound
Mental health care and support starts at birth, and the first five years of your child’s life are crucial to their development. The Early Childhood Wraparound program uses the same process as the FAST Forward program and helps identify your strengths and your most urgent needs to develop a long-term plan to improve mental health outcomes for you and your child. The program is collaboration based and results driven, ensuring you and your child’s needs are met.
TrECC
For youth, 21 years old and younger who are transitioning in or out of residential or psychiatric care centers, it can be difficult to find help that meets their needs. These transitions can be challenging and critical, which is why Connected Families NH is here to help you build your support system as you take your next steps. By identifying your strengths and needs you can develop a plan that supports returning to your community of choice.
During the next 3 – 9 months of this transition, your care coordinator will help you work with the professionals you already trust as well as manage referrals to other services that match your goals.
Our programs aren’t intended to replace any existing services. Instead, we work with you, your natural supports, and the professionals you trust to boost your independence. Our mission is to ensure you are not alone as you and your family tackle this transition.
